(we provide a detailed bill for those planning to use their out of network benefits to get reimbursed by their insurance)
- Initial Phone Consult fee: No Charge
- Brief initial consultation for prospective patients
- Discuss reason for appointment request with Dr. Moideen
- Explore if the practice structure and policy will meet your current needs
- If you wish to continue with us, we can schedule an initial evaluation
- Initial Evaluation fee: $425.00 Time: 70 minutes
- Purpose: Comprehensive Initial Psychiatric evaluation with medication management
- Follow up appointments for Established Patients:
- Time: 30-minute $225
- Time: 45 minutes $325
- Purpose: medication management with therapy

Frequently Asked Questions
When it is time for your appointment, you will log on to the Patient Portal here. Once here, you will enter your name to check in. Please fill out the initial questionnaire before the appointment starts.
Please make sure your device is charged or plugged in to a charger before your appointment begins. In addition, please make sure you have headphones or are in a quiet environment in order to ensure quality communication for both parties during your appointment.
Your first appointment will be about 70 minutes long. We will discuss your medical and psychiatric history, current symptoms and treatment options
Each appointment will differ. During your virtual visit, you may discuss symptoms that you are having and potential treatment.
